Connected Communities has grown into one of the nation’s largest providers of housing-based supportive services since its founding in 2017. Over the past six years, we have nearly doubled in size, now serving over 230 communities across 18 states and the District of Columbia. In partnership with more than 3,000 service providers, we connect residents with programs, services, and resources that drive meaningful and measurable improvements in their quality of life.
